PHARMACIST - POOL, FHP - ISC MAIL ORDER PHARMACY

Location: Menomonee Falls, WI (US). Position is hybrid, full‑time (0.001 FTE) with a standard 40‑hour work week (Shift 1).

Responsibilities

• Responsible for all aspects of pharmacotherapy on an assigned patient care unit.
• Provide clinical support and distribution services in coordination with physicians and nursing staff to deliver quality, cost‑effective drug therapy.
• Perform other duties as assigned.

Requirements

• Minimum of one year hospital pharmacy experience or three years of other pharmacy experience (preferred).
• Bachelor’s degree in Pharmacy or Pharm.D. from an ACPE‑certified school (required).
• Successful completion of an ASHP‑accredited PGY1 residency (preferred).
• Active Wisconsin Pharmacist License required or eligible and must obtain within 3 months.
• Demonstrated effective use of computers and applications (e.g., MS Word, Excel, Outlook) and strong communication skills.
• Active membership in the Pharmacy Society of Wisconsin (preferred).
• Active membership in the American Society of Health‑System Pharmacists (ASHP) or other national pharmacy organizations as approved (preferred).
