PHARMACIST, FHP - ISC MAIL ORDER PHARMACY

Location: Menomonee Falls, WI (US). Hybrid schedule. Full‑time (40 hrs/week, Shift 1).

About the role

Using independent decision making and critical thinking skills, the Pharmacist promotes medication use that is safe, effective and considers the costs and benefits of therapy to maximize value. The Pharmacist interacts with other members of the healthcare team and with patients to proactively address medication therapy issues. The Pharmacist ensures patient care is maintained through compliance with the department's quality improvement plan, and the Joint Commission/Center for Medicare/Medicaid standards for pharmaceutical services.

Requirements

• Minimum of 1 year hospital pharmacy experience or 3 years other pharmacy experience (preferred).
• Bachelor's degree in Pharmacy or Pharm.D. from an ACPE‑certified school of pharmacy (required).
• Active Wisconsin Pharmacist License (or eligibility with license obtained within 6 months).

Qualifications

• Successful completion of an ASHP‑accredited PGY1 or specialty residency (preferred).
• Active membership in the Pharmacy Society of Wisconsin (preferred).
• Active membership in the American Society of Health System Pharmacists (ASHP) or other national pharmacy organizations approved by Pharmacy Administration (preferred).

Skills

• Effective use of computers and applications (e.g., MS Word, Excel, Outlook) for practice and communication.
