Pharmacist - Medication Therapy Management

Responsibilities

• Perform telephonic comprehensive medication reviews, including medication reconciliation and clinical assessments.
• Engage and HIPAA authenticate members for comprehensive medication reviews.
• Update member mailing addresses, document medication allergies and side effects, and verify medication lists.
• Identify, document, and facilitate resolution of medication related problems.
• Create a written summary (patient takeaway) of each member interaction at an appropriate level of health literacy.
• Create faxed materials for prescriber offices to facilitate resolution of medication related problems.
• Build and maintain positive relationships with internal and external customers, including physicians and nurses.
• Complete internal training to utilize behavioral modification and interviewing skills.

Qualifications

• Previous pharmacy experience delivering MTM, specifically using the Outcomes MTM Connect platform.
• 7 months within the MTM Ops department and meets expectations in all core metrics.
• Previous experience in a call center environment.
• Experience maintaining focus and meeting goals in a work from home environment.
• Excellent spoken and written communication skills.
• Basic computer skills and proficiency with Microsoft Office products.
• Exceptional prioritization and organizational skills.
• Acts with integrity and uses sound judgment in dealing with confidential information.
• Resides and has an active RPh license in the state of PA, IL, or FL.
