About the Role

Provide pharmaceutical care services in the ambulatory care setting utilizing approved protocols and following established policies. Deliver customer-focused clinical pharmaceutical care, including primary care, anticoagulation services, and chronic condition management. Work under the direction of the Program Director of Medication Management in collaboration with the multidisciplinary team to identify clinical interventions and ensure medication safety across the continuum.

Serve as a medication information specialist by disseminating pertinent information to physicians, patients, nursing, and other clinical staff. Participate in patient and provider educational programs to positively impact patient compliance and provider prescribing practices. Provide pharmaceutical care through continual monitoring of patients’ drug therapies and accepting responsibility for outcomes.

Maintain the security of clinic medication supplies and effectively utilize pharmacy technology-driven systems to improve patient safety and streamline workflow. Responsible for quality reports, medication utilization evaluations, performance improvement activities, and other assigned projects, adhering to set deadlines. All efforts shall be coordinated with and have the approval of the designated physician(s) and/or Program Director of Medication Management.

Responsibilities

  • Maintain security of clinic medication supplies.
  • Utilize pharmacy technology to enhance patient safety and workflow efficiency.
  • Prepare quality reports, medication utilization evaluations, and performance improvement activities.
  • Complete assigned projects and duties within established deadlines.

Requirements

  • PharmD degree from an accredited school of pharmacy recognized by the California State Board of Pharmacy.
  • Current California Registered Pharmacist license.
  • Current BCLS Healthcare Provider CPR Card from the American Heart Association.
  • Completion of an ambulatory care residency and/or advanced certification in a pharmacy specialty area.
  • Minimum of three (3) years of experience as a California Registered Pharmacist in a hospital or ambulatory care setting with a clinical background in a specialty area.

Skills

  • Advanced pharmacokinetics and clinical knowledge of disease states in the patient population.
  • Ability to apply clinical knowledge in an ambulatory care setting.
  • Critical thinking and effective education skills.
  • Knowledge of medication-related policies/protocols, clinical trials, and evidence-based practices.
  • Excellent computer, verbal, and written communication skills.
  • Ability to prioritize tasks and resolve patient needs considering age and biophysical, psychological, educational, and environmental factors.

Preferred Qualifications

  • Experience with design of pharmacy systems and equipment specific to medication administration.
  • Bilingual proficiency.

$140,400.00 - $231,504.00 per year.
