Clinical Pharmacist Manager

Overview

The Clinical Pharmacist Manager will work directly with Members, providers and healthcare professionals within the CINQCARE network. The Clinical Pharmacist will support peer to peer consults for Clinical Pharmacy questions, Clinical Pharmacy case review, Clinical Pharmacy education and training, and support overall quality of care and performance outcomes for patients and Value-Based Provider Groups. They will be an integral member of the Clinical Operations Team, using advanced data and analytics to improve polypharmacy, medication optimization for high-risk patients, medication adherence, and support to providers and patients in disease management.

Administrative Duties

  • Daily oversight and management clinically and operationally of clinical pharmacy staff.
  • Training and mentoring of clinical pharmacy staff including a team of clinical pharmacists and pharmacy technicians
  • Manage an organization-wide medication adherence program including devising strategies and work-flows for meeting and tracking performance goals.
  • 0.2 FTE is allocated to administrative tasks and 0.8 FTE allocated to direct patient care responsibilities

Direct Care Duties

  • Support day to day requirements for the CINQCARE Clinical Pharmacy Program
  • Complete provider requested peer-to-peer clinical pharmacy consults. This may include but is not limited to:
    • Medication questions
    • Clinical pharmacy medication management guidance
    • Drug-Drug interactions and/or contraindications
    • Behavioral Health medication guidance and regimen optimization
  • Complete high-risk patient case pharmacy reviews
  • Evaluation medication profiles and determine if changes are warranted to optimize medication therapy and reduce potential for adverse drug reactions
  • Recommend alternative medications, dosing or medication discontinuation, as appropriate
  • Review therapy plans with providers
  • Support interactive patient visits for Medication Therapeutic Management including medication review, adherence and education
  • Participate in IDT Clinical rounds with Providers to provide therapeutic insights
  • Engage directly with providers and patients to support clinical pharmacy interdisciplinary outcomes
  • Develop and support provider education didactics for Clinical Pharmacy Topics of Interest
  • Present evidence-based guidelines, quality improvement, and savings opportunities to the CINQCARE network of provider partners and internal providers as appropriate

Qualifications

  • Education: PharmD from an ACPE accredited School of Pharmacy
  • License: Active, unrestricted, pharmacist license; preferred licensure in NY, MD, WV, TX, FL, CA, IL, IN, or NJ.
  • Experience: At least 2 years of clinical pharmacy experience.
  • Strong decision-making skills and ability to work independently to meet deadlines.
  • Knowledge related to Part B Drugs (including but not limited to immunotherapy, biologics and chemotherapeutics) and behavioral health medications (anti-psychotics, long acting injectables and controlled substances) is preferred but not required

  • Communication: Excellent verbal, written communication and presentation skills; ability to clearly articulate and present concepts and models in an accessible manner to CINQCARE's team, investors, partners, and other stakeholders. Proficiency in all Microsoft Office applications.
  • Relationships: Ability to build and effectively manage relationships with business leaders and external constituents.
  • Culture: Good judgement, impeccable ethics, and a strong team player; desire to succeed and grow in a fast-paced, demanding, and entrepreneurial Company.

Working Environment & Physical Requirements

  • Work is performed indoors in a setting with air conditioning and artificial light.
  • Travel to and work in offices or other environments is required.
  • In this position you will need to communicate with customers, vendors, management, and other co-workers in person and over devices, sometimes with people who are agitated.
  • Regular use of the telephone and e-mail for communication is essential.
  • Sitting for extended periods is common.
  • Must be able to receive ordinary information and to prepare or inspect documents.
  • Lifting of up to 10 lbs. occasionally may be required.
  • Good manual dexterity for the use of common office equipment such as computer terminals, calculator, copiers, and FAX machines.
  • Good reasoning ability is important. Able to understand and utilize management reports, memos, and other documents to conduct business.
