About the Role
NYC Health + Hospitals/North Central Bronx (NCB) is a 215-bed community hospital specializing in women’s and children’s services, Behavioral Health acute and ambulatory care, and community-based outpatient services. As a Collaborative Drug Therapy Management Pharmacist, you will serve as a liaison between patients, ambulatory, clinical, and pharmacy teams to ensure efficient drug therapy and positive patient outcomes. This role is crucial in disease state management, medication optimization, and enhancing the patient experience.
Responsibilities
- Provide consulting services to patients on medication use, adherence, and drug interactions.
- Partner with the Office of Ambulatory Care and collaborating physicians to deliver patient care.
- Administer preventative healthcare services, including immunizations.
- Manage drug therapy for patients with specified disease states in an ambulatory clinic setting.
- Adjust and manage drug regimens based on patient-specific written orders or protocols.
- Evaluate patients to ensure appropriate medical regimens and detect adverse drug effects.
- Collaborate with ambulatory care and clinical teams to educate patients on pharmacy services.
- Utilize facility and enterprise formularies, manage restricted drugs, and handle prior authorizations for insured patients.
- Establish efficient workflows with clinic providers and the pharmacy team.
Qualifications
- Doctor of Pharmacy (Pharm.D.) degree or equivalent.
- Valid New York State license to practice pharmacy.
- New York State certification to practice Collaborative Drug Therapy Management (CDTM).
- Completion of a pharmacy residency training program in a clinical/hospital setting.
Pay
Salary offered: $154,097.
