Job Summary
The Medication Management Clinic (MMC) is an ambulatory care pharmacist-led clinic that partners with primary care and specialty physicians to assist patients navigating the pharmacy system. MMC pharmacists work at the top of their license through collaborative practice agreements to manage drug therapy including prescriptive authority for various disease states.
Job Requirements
Minimum Education: B.S. Pharmacy or Pharm D, or Master of Pharmacy
Continuing education as required for licensure by TSBP
Minimum Work Experience: Previous hospital experience with clinical responsibilities
Required Licenses/Certifications: Active registration as a licensed pharmacist in the state of Texas, BLSTSBP Preceptor required within 12 months of hire
Required Skills, Knowledge, and Abilities:
Assess, document, and perform physician consultations, excluding total parenteral nutrition, but including all medication dosing consults
Provide education presentations to pharmacy staff and other staff upon request
Ability to effectively direct and/or supervise personnel as appropriate
Ability to handle confidential information
Ability to maintain professional appearance and decorum at all times
Ability to demonstrate excellent oral and written communication skills
