Patient Care Management
Provides supervision of daily clinic workflow and clinical practice of clinic staff (RN and support staff).
Demonstrates the ability to make decisions by integrating didactic knowledge with practical experience.
Performs continual assessment, outcomes identification, planning, implementation, and evaluation of the care plan for anti-coagulated patients.
- Routine clinical management activities will include: INR result interpretation; warfarin dosage adjustment; follow up INR and/or other testing recommendations; ongoing assessment of anticoagulation efficacy; identification/management of potential complications; parenteral bridging assessment; peri-procedural anti-coagulation care planning; and continued individualized patient orientation to the anti-coagulation clinic.
- Ensures compliance with collaborative agreement directing the anti-coagulation therapy process in the clinic: dose adjustment, ordering of labs, prescribing of warfarin, and ongoing monitoring of patient response to therapy.
- Reviews and cosigns visit progress notes of clinic nurses, pharmacy residents and students.
- Records notes, assessment data in to EPIC system, creates appropriate visit notes and after visit summary.
- Continually assesses patient and family educational needs with regard to anti-coagulation.
- Demonstrates an awareness of environmental, cultural, social, and/or financial factors on the anti-coagulation care of patients and their families.
- Provides anti-coagulation care in a manner that incorporates patient satisfaction principles and cost-effective care.
- Develops relationships with patients and families that maintain and communicate trust and respect.
- Communicates effectively in the exchange of information.
- Demonstrates the ability to act as a patient advocate.
- Utilizes SEH multi-disciplinary resources for medical, financial, social and/or other assistance; identifies patient specific resources to improve anti-coagulation adherence; and information provision in the format that best allows patients to manage their specific anti-coagulation care needs.
Supervises and Directs clinic nurses and support staff
- Provides active direct supervision and mentoring of clinic nurses and support staff as guided by ICARE values.
- Makes sure timely completion of tasks, redirection of staff to meet patient needs, and encourages use of the AIDET model in interaction with staff, patients and visitors.
- Reports behavior not responsive to direct intervention to the leadership team.
Drug Information / Staff Development
- Provides accurate, adequate and timely drug information to the medical staff, nursing staff, and patients.
- Serves as a preceptor or provides advice to Pharmacy Residents and students.
- Develops staff education programs.
Maintains Professional Competency
- Maintains licensure.
- Completes all departmental competency/skills assessment requirements.
- Maintains current knowledge of pharmacy practice.
- Seeks to develop and maintain skills in areas of specialized practice.
