Pharmacist - Ambulatory Care Clinic - Multispecialty

About the role

The Multispecialty Ambulatory Care Clinical Specialist at Cedars-Sinai Medical Center in Los Angeles, California is an Advanced Pharmacist Practitioner who collaborates with a multidisciplinary team to deliver comprehensive medication management for patients across a range of specialty ambulatory clinics.

Responsibilities

• Supports patients throughout their treatment journey through medication reconciliation, assessment of therapy appropriateness and tolerability, patient education on specialty medications and infusions, longitudinal follow-up and outcomes monitoring, and support with medication access.
• Achieves positive patient outcomes through disease management guided by established protocols, promotes adherence, and enhances satisfaction among both patients and providers as evidenced by outcome evaluations.
• Follows updated clinical guidelines and Cedars-Sinai's P&T-approved protocols to initiate, adjust, or discontinue therapies, order and interpret lab results, and authorize medication refills.
• Educates patients about specialty medications and serves as a clinical resource for physicians and support staff.
• Helps develop and maintain clinical pathways, best practices, protocols, competency assessments, and supports ongoing quality improvement efforts.

Qualifications

• Educational Requirements: Graduate Degree from an accredited School of Pharmacy – Minimum Completion of a PGY1 ASHP accredited residency program (or equivalent work experience), Completion of a PGY2 ASHP accredited residency program in ambulatory care – Preferred, Licenses and Certifications: California Pharmacist license, Advanced Pharmacist Practitioner (APP) License – Required or must be eligible for and obtain within twelve (12) months of hire, Board certification in relevant practice area (e.g., BCACP, BCPS) – Required or must be eligible for and obtain within eighteen (18) months of hire, Professional certificate in relevant practice area(s) – Preferred

What Else Are We Looking For?

• Clinical Practice Experience: Previous experience in applicable specialty areas or infusion therapy management in an ambulatory care setting is preferred.
• Patient Experience: Possesses excellent patient service skills prioritizing patient care needs to support safe effective medication use.

• Analytical: Apply critical thinking to analyze and interpret information and/or data. Ability to analyze qualitative and quantitative information for decision support.
• Analytical: Demonstrates good decision-making/problem solving skills. Exhibits good clinical reasoning skills and judgments in managing patients with the highest medical complexity/acuity.
• Communication: Ability to convey and/or receive written/verbal information to/from various audiences in different formats. Communicates effectively and engages in positive inter/intra departmental interaction with peers, physicians, staff and patients.
• Documentation: Demonstrates the ability to accurately and promptly document all clinical interventions, assessments, and therapy changes in the patient’s Electronic Health Record (EHR).
• Project Management: Initiate, plan, design, implement, and monitor project(s) Ability to manage projects, establish timeliness, analyze data relevant to areas of responsibility. Ability to function independently, ability to plan, organize, direct, monitor and evaluate assigned work.
• Budgetary & Financial: Possesses fiscal management skills and proven track record to prepare, monitor and control budgetary expenses and generate revenue.
• Regulatory: Ability to interpret and apply knowledge of State, Federal and Agency standards to comply with regulations.
• Technical: Ability to learn and adapt to the use of software, technology, and automated information systems in daily practice.
• Physical Demands: Frequent sitting; walking, bending; reaching, occasional pushing, pulling, lifting of materials up to 50 pounds. Responsiveness to auditory alarms and communication devices including telephones, pagers, fire alarms, and mechanical failure alarms. Sufficient mobility to access equipment, patients, and other customers. Able to read information on medication labels.
