Ambulatory Care Pharmacist

Responsibilities

• Properly assesses patients' pharmaceutical needs and provides pharmaceutical care.
• Addresses any medication access issues and provides care coordination if needed.
• Buils and maintains partnerships with patients and other healthcare providers to strengthen the multidisciplinary healthcare team.
• Stays updated on therapy guidelines and advancements in Ambulatory Care Pharmacy.
• Properly counsels patients and health care professionals regarding drug therapy.
• Orders, interprets, and monitors laboratory testing as necessary.
• Maintains medication therapy adherence.

Qualifications

• Education: Bachelor of Science degree in Pharmacy or Pharm D.
• Licensure: Current state pharmacist licensure in good standing or licensure eligibility within 90 days of hire.
• Immunization certification.
• Minimum Work Experience: Residency training (PGY-1) preferred or previous Ambulatory Care experience or 1-2 years of retail experience.
