Ambulatory Care Pharmacist

Responsibilities

• Properly assesses patients' pharmaceutical needs and provides pharmaceutical care.
• Addresses any medication access issues and provides care coordination if needed.
• Buils and maintains partnerships with patients and other healthcare providers to help build a strong multidisciplinary healthcare team that works to improve patient outcomes.
• Stays up-to-date on therapy guidelines and advancements in Ambulatory Care Pharmacy.
• Properly counsels patients and patients' families, and appropriate health care professionals regarding drug therapy.
• Orders, interprets, and/or monitors laboratory testing if necessary.
• Makes appropriate interventions to improve patient care based on current guideline-directed therapy.
• Performs preventative care (e.g., screening and administering vaccinations) or wellness screenings.
• Uses, is proficient in the operation, and documents within the pharmacy computer system and the electronic medical record.
• Obtains pertinent patient medication/medical information.
• Provides current and accurate drug information to patients and health care professionals.
• Communicates effectively with patients, health care professionals, and coworkers.
• Dispenses and/or administers medications to patients in accordance with ARH policies, including but not limited to, all routes of immunizations, naloxone, and injectable medications where permitted by law.
• Completes and properly documents medication therapy management interventions.
• Ensures compliance with company and legal policies, procedures, and regulations.
• Ensures confidentiality of information, documentation, and records.
• Supports the pharmacy manager or specialty pharmacist as needed.

Qualifications

• Education: Bachelor of Science degree in Pharmacy or Pharm D
• Current state pharmacist licensure in good standing or licensure eligibility in state of employment within 90 days of hire
• Immunization certification
• Minimum work experience: Residency training (PGY-1) preferred or previous Ambulatory Care experience or 1-2 years of retail experience

• Board certification, clinical experience, and continuing-education certification programs are encouraged in Ambulatory Care Pharmacists participating in direct patient care
