This role is based in Rochester, New York, and is a full-time position within the Pharmacy department at Strong Memorial Hospital, working evening shifts.

Responsibilities

  • Provide comprehensive clinical pharmacy services to optimize safe, effective, and cost-effective drug therapy.
  • Serve as a member of interdisciplinary patient care teams, offering pharmacotherapeutic and pharmacokinetic consults.
  • Deliver patient and provider education, and develop best practice guidelines and policies related to pharmacotherapy.
  • Lead departmental initiatives in your specialty area, including organizing educational programs for colleagues, providers, and trainees.
  • Participate in inter-disciplinary patient care rounds and provide consultative input on drug therapy.
  • Monitor assigned patient populations for clinical progress, including renal/hepatic function, culture results, and laboratory tests; recommend therapy adjustments as needed.
  • Provide patient education, discharge counseling, and authorize/verify orders; supervise drug distribution when required.
  • Document clinical pharmacy activities in the medical record or other systems.
  • Collaborate with clinicians and healthcare providers to implement innovative disease or medication management programs.
  • Develop and maintain clinical guidelines, critical pathways, disease management, and drug use programs.
  • Mentor, coach, and model professional behavior for pharmacists in the department.
  • Supervise technicians to ensure appropriate workflow and conduct quality assurance, medication use evaluations, or research to document the value of clinical pharmacy services.
  • Participate in departmental quality assurance processes, including reporting and following up on medication incidents, reconciling medication administration records, and cost-saving initiatives.
  • Commit to training future pharmacists by serving as a preceptor for residency programs and contributing to student training experiences (IPPE and APPE rotations).
  • Provide didactic lectures to medical center divisions and departments, including nursing, medical/surgical residents, and allied health students.
  • Resolve problems through clear and direct communication with pertinent staff; escalate issues to supervisors when necessary.
  • Identify patient care and system-level problems; participate in interdisciplinary teams or revise policies, procedures, or treatment guidelines as needed.
  • Hold active membership and participate in local, regional, and national professional organizations in clinical pharmacy and/or specialty areas.
  • Present clinical education or research results at local, regional, and/or national meetings; provide continuing professional education to peers.
  • Serve on departmental, hospital, or medical center committees or task forces.

Minimum Education & Experience

  • Doctor of Pharmacy degree required.
  • Specialty residency training preferred, or equivalent combination of education and experience.
  • New resident graduates or clinical specialists in their first 3–5 years of practice preferred.

Licenses and Certifications

  • New York State Pharmacy License required upon hire.
  • Board certification in the most relevant specialty area required within 2 years.

Pay

The compensation range for this role is $126,256.00 to $170,456.00 annually. Individual salaries will be set within this range based on market data, education, experience, qualifications, internal equity, and other relevant factors.
