About the role
Pharmacist 2 within an integrated healthcare team, providing patient-centered clinical pharmacy services in inpatient and outpatient settings.
Responsibilities
- Verify and dispense medication orders.
- Perform drug interaction and allergy screenings.
- Ensure accurate dosing and counsel patients on medication use, side effects, and adherence.
- Conduct medication reconciliation during care transitions.
- Support formulary management and serve as a drug information resource for healthcare providers.
- Prepare sterile and non-sterile compounds, including IV admixtures, following safety standards.
- Utilize electronic medical records and pharmacy systems for documentation and workflow efficiency.
- Participate in quality improvement, medication safety reviews, and ensure compliance with regulatory standards.
- Contribute to the development of clinical protocols, order sets, and therapeutic guidelines.
- Support staff education, precept students or residents, and engage in interprofessional rounds.
Qualifications & Skills
- Strong knowledge of medication therapy management, drug interactions, and clinical guidelines.
- Proficiency with pharmacy information systems, automated dispensing, and sterile compounding.
- Excellent communication skills with a focus on health literacy.
- Ability to collaborate effectively within a multidisciplinary team.
- Commitment to quality, safety, and continuous improvement.
- Experience in clinical projects, research, or advanced pharmacy services is a plus.
