PH RN-Home Visiting Program

About the role

The Northeast Health District is seeking a compassionate and dedicated Public Health Nurse to join our Home Visiting Program. This is a part-time, hourly position working up to 29 hours per week.

Responsibilities

• Deliver case management and nursing care according to public health protocols.
• Provide services focused on health promotion, illness prevention, and decreasing infant mortality.
• Congduct home visits and community outreach services to high-risk perinatal women and infants.
• Perform physical assessments, obtain vital signs, conduct fetal monitoring, and perform basic laboratory screenings.
• Provide breastfeeding education and counseling and refer clients to appropriate social and health services.
• Maintain accurate records of patient care, services, and responses to treatment.
• Collaborate with healthcare professionals and community partners.
• Participate in emergency preparedness and response activities.

Requirements

• Associate degree in Nursing from an accredited college or university and current Georgia Registered Professional Nurse (RN) license.
• CPR certification and completion of the Public Health Nurse (PHN) course within one (1) year of hire.
• A valid driver's license and reliable transportation are required.

Qualifications

• Minimum qualifications: Associate degree in Nursing, current Georgia RN license, CPR certification, and completion of PHN course within one year of hire.
• Preferred qualifications: BSN, experience in labor and delivery, prenatal care clinics, and home visitation.

Skills

• Bilingual in Spanish.
