In-Home Visit Nurse

Phoenix Home Care & Hospice is a mission-driven company dedicated to supporting clients in overcoming daily healthcare challenges. They are currently seeking a motivated and compassionate Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join their in-home team.

Responsibilities

• Complete admission assessments and reassessments for the in-home caregiver population.
• Provide nurse visits for adult and senior populations, including taking vitals, managing medications, performing monthly diabetic nail checks, and conducting labs.
• May provide supervisory visits for caregiver services.
• Provide instruction and education on medication management.
• Show good time management, attention to detail, and self-starting abilities.

Requirements

• Active Registered Nurse or Licensed Practical Nurse license in the state of Missouri.
• Home care experience preferred but not required.
• Strong organizational and self-management skills.
• Strong and compassionate customer service skills.
• A valid driver’s license.
