Become a part of our caring community Iowa City / North Liberty Patients As a CenterWell Skilled Home Health nurse, reporting to the Clinical Manager, you will provide personalized, one-on-one care that helps patients regain independence in the comfort of their homes. Working with a dedicated team of physicians and clinicians, you'll manage care plans that support recovery and help patients get back to the life they love. For this fulltime position providing skilled home care the patients are located in our South Region including the Iowa City, North Liberty areas. Responsibilities

• Provide personalized, one-on-one care in patients' homes.
• Administer medications, perform wound care, and conduct patient assessments.
• Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
• Maintain accurate records of patient care and coordinate with other healthcare professionals.
• Report patient care and condition progress to patient's physician and Clinical Manager.
• Oversee the work of Home Health Aides and monitor their appropriate completion of documentation.

Requirements

• Med surg, ICU, ER, or acute experience.
• Current and unrestricted Registered Nurse licensure.
• CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Valid driver's license, auto insurance and reliable transportation.
• Live within coverage area of North Liberty / Iowa City patient census.

Preferred Qualifications

• Home Health experience.

Pay

Pay Range $49.00 - $69.00 pay per visit/unit $77,200 - $106,200 per year base pay
