Registered Nurse, Home Visits

About the role

Baptist Home Health Care by BAYADA is seeking a compassionate and dedicated Registered Nurse to join their team. This position serves adult and geriatric clients across Nassau County and Northern Duval through per visit home health care services. Previous experience in med-surg and ICU settings is highly valued.

Responsibilities

• Follow a designated care plan based on patient needs
• Make home visits within designated territories
• Administer medications, perform wound care, treatments, and procedures
• Maintain vigilance over clients' conditions and report any changes to appropriate parties
• Execute and document doctors' orders accurately
• Conduct regular client assessments
• Accurately document observations, interventions, and evaluations related to client care
• Participate in case management and coordination

Requirements

• A current license as a Registered Nurse in Florida
• A minimum of one year of recent, verifiable clinical (medical/surgical) experience
• Prior home care experience is preferred but not required
• Graduation from an accredited and approved nursing program
• Ability to work independently and manage time effectively
• Strong interpersonal skills
• Strong computer skills, especially with electronic medical records (EMR)
