EverCare Mobile Health is seeking a Per Diem Telehealth Facilitator to support patients and providers throughout the Magic Valley area. This position works directly inside long-term care and residential facilities, helping patients connect with EverCare mental health providers through telehealth. This is an as-needed position, with hours expected to average approximately 10–25 hours per week, depending on patient volume, provider schedules, and facility needs. Hours are not guaranteed and may fluctuate from week to week. What You'll Do As a Telehealth Facilitator, you will serve as the on-site connection between patients, facility staff, and EverCare's remote healthcare providers.
