Telehealth Provider, NP, Remote, Per Diem
About the Role

As an Advanced Practice Provider (APP) in Telehealth, you’ll deliver diagnostic patient care primarily through virtual, remote consultations via video conference or telephone. This PRN role serves as the dispositional authority for after-hours and weekend clinical calls, assessing, diagnosing, and treating geriatric patients while documenting their physical and psychosocial health status.
You’ll work on an as-needed basis, including nights, weekends, and holidays, with a minimum requirement of 24 hours per month (including 4 hours per weekend) to maintain per diem status. This role also involves covering shifts for PTO, LOA, and unexpected absences, with some shifts planned in advance and others requiring minimal notice.
Responsibilities
- Assess acute and non-acute clinical problems through virtual video conference or telephone.
- Perform and document physical evaluations, patient histories, and analyze trends in patient conditions.
- Develop, document, and implement patient management plans based on findings.
- Aid in creating plans of care, including health education, physician referrals, case management, and patient/family counseling.
- Evaluate the need for immediate nursing intervention, consultation, or referral and facilitate necessary care.
- Plan patient care based on knowledge of the patient population, cultural background, and support systems.
- Serve as a patient advocate, collecting comprehensive health data and determining appropriate health maintenance methods.
- Confer with patients’ primary care providers (PCPs) and other medical providers to report health data and ensure compliance with guidelines.
- Consult with patients and families on health outcomes and maintain or improve positive health habits.
- Lead efforts to ensure optimal patient outcomes through telemedicine, collaborating with on-call PCPs as needed.
- Implement appropriate protocols to attain expected clinical outcomes and evaluate progress.
- Document assessments, interventions, and progress in a clear, translatable format.
- Enhance telehealth practices and systems using data demonstrating program effectiveness.
- Communicate using various formats, tools, and technologies to build professional relationships and deliver care.
- Utilize resources to plan and provide safe, effective, and financially responsible services.
- Provide extraordinary customer service and professionalism to all internal and external customers.
- Participate in clinical rounds, conferences, risk and quality management programs, and relevant meetings.
- Adhere to departmental goals, standards of performance, regulatory compliance, and quality patient care policies.
- Practice in accordance with a written or electronic practice agreement.
- Perform other duties as assigned or modified at the manager’s discretion.
Requirements
- Bachelor’s degree in Nursing (BSN) and graduation from a school of nursing for Advanced Practice Nursing with certification in area of specialty; Master’s degree in Nursing required.
- Valid, active Registered Nurse license.
- Board certification by AANP or ANCC.
- Basic Life Support (BLS) certification from the American Heart Association or American Red Cross (within first 90 days of employment).
- Current DEA number for schedule II-V controlled substances (may be required based on state of practice).
- Multi-state licensure, including Florida and at least 2 licenses in the following states: VA, GA, KS, MI, MO, NC, OH, PA, SC, TN, TX, IL, KY, LA.
- Minimum of 3 years’ acute/primary care/urgent care clinical work experience; geriatric patient experience highly preferred.
- Minimum of 2 years’ telehealth work experience preferred.
- Expert-level business acuity and knowledge of medical practices to function independently and in collaboration with licensed physicians.
- Technical capability to conduct telemedicine visits in accordance with state and federal regulations.
- Ability to demonstrate excellent clinical judgment, problem-solving, and prioritization under pressure.
- Strong communication and collaboration skills with physicians, patients, and multidisciplinary teams.
- Willingness to travel locally within South Florida (up to 10% of the time) for meetings and trainings.
- Required availability to work evenings, overnights, weekends, and three holidays per calendar year.
- Proficient in Microsoft Office Suite (Excel, Word, PowerPoint, Outlook) and other required systems.
- Spoken and written fluency in English; bilingual (Spanish/Creole) a plus.
Pay
The pay range for this position is $53.40 - $76.33 per hour. Final compensation will depend on experience, education, geographic location, and other relevant factors. This position may also be eligible for bonuses or commissions.
