Responsibilities
- Affirms and supports individuals served.
- Develops, implements, and documents individual treatment support plans.
- Engages in daily tasks with individuals served, including (re)habilitative interventions, behavioral interventions, and monitoring.
- Schedules appointments and monitors progress.
- Provides transportation for individuals served.
- Assesses physical and emotional status and reports changes to supervisor.
- Maintains program site safety and cleanliness.
- Participates in daily living and cleaning tasks, including cooking, meal planning, grocery shopping, and household chores.
- Complies with company policies and procedures, including safe work practices and exposure control.
- Administers and documents medication according to MAP requirements.
- Completes various paperwork and logs.
- Provides crisis intervention as needed.
- Provides current contact information to Relief Pool Coordinator or Human Resources staff.
- Performs other related duties as required.
Qualifications
- Minimum of 2 direct care shifts per month.
- One to three months experience in a human services setting, or equivalent education.
- A high school diploma or equivalent.
- A valid driver’s license with at least one year of experience.
- Ability to lift at least 25 pounds using proper techniques.
- Ability to operate a computer and other office equipment.
- Ability to sit, reach, climb stairs, and maneuver through narrow spaces or hallways.
- Ability to assist clients with tasks of daily living.
- First Aid certification within two weeks of hire.
- CPR certification within two weeks of hire.
- MEDICATION ADMINISTRATION PROGRAM (MAP) certification within 150 days of hire.
