Responsibilities

  • Affirms and supports individuals served.
  • Develops, implements, and documents individual treatment support plans.
  • Engages in daily tasks with individuals served, including (re)habilitative interventions, behavioral interventions, and monitoring.
  • Schedules appointments and monitors progress.
  • Provides transportation for individuals served.
  • Assesses physical and emotional status and reports changes to supervisor.
  • Maintains program site safety and cleanliness.
  • Participates in daily living and cleaning tasks, including cooking, meal planning, grocery shopping, and household chores.
  • Complies with company policies and procedures, including safe work practices and exposure control.
  • Administers and documents medication according to MAP requirements.
  • Completes various paperwork and logs.
  • Provides crisis intervention as needed.
  • Provides current contact information to Relief Pool Coordinator or Human Resources staff.
  • Performs other related duties as required.

Qualifications

  • Minimum of 2 direct care shifts per month.
  • One to three months experience in a human services setting, or equivalent education.
  • A high school diploma or equivalent.
  • A valid driver’s license with at least one year of experience.
  • Ability to lift at least 25 pounds using proper techniques.
  • Ability to operate a computer and other office equipment.
  • Ability to sit, reach, climb stairs, and maneuver through narrow spaces or hallways.
  • Ability to assist clients with tasks of daily living.
  • First Aid certification within two weeks of hire.
  • CPR certification within two weeks of hire.
  • MEDICATION ADMINISTRATION PROGRAM (MAP) certification within 150 days of hire.
