Per Diem Primary Care Provider (Casual Employee)

Role Overview A primary care clinician providing in-office and telehealth services, conducting acute, chronic, and wellness visits without panel-building responsibilities. Key Responsibilities Assess and treat patients with diverse healthcare needs, utilizing clinical training for in-office procedures.Collaborate with in-person and virtual teams through daily huddles and communication channels.Deliver high-quality, evidence-based primary care in a fast-paced, patient-centered environment. Qualifications & Skills Minimum 2 years of primary care experience.Licensed or eligible for licensure in Connecticut.Board certified or board eligible in Family Medicine or Internal Medicine.Completed accredited NP or PA program with national certification.Strong communication skills, ability to build relationships, and proficiency with technology.Patient-centered approach, adaptable, and receptive to feedback.
