Position Summary
BrightStar Care of Acton–Andover–Lowell is seeking Per Diem Physical Therapists (PT/RPT) to complete assessment-only visits for skilled home health patients. This role is ideal for clinicians looking for flexible scheduling, meaningful one-on-one patient care, and limited ongoing treatment responsibilities.
Responsibilities
- Conduct initial and follow-up assessments in accordance with physician orders and clinical guidelines
- Evaluate patient functional abilities and determine care needs
- Establish plan of care and provide appropriate clinical recommendations
- Educate patients and caregivers on safety, mobility, and home exercise strategies
- Complete timely and accurate documentation in compliance with state and regulatory standards
- Collaborate with nursing and care teams to support optimal patient outcomes
Qualifications
- Graduate of an accredited Physical Therapy program
- Active MA PT license
- Experience in home health, skilled nursing, or sub-acute rehab preferred, not required
- Strong clinical judgment, documentation accuracy, and communication skills
- Ability to work independently and travel within the local service area

Pay
$65 to $90 per visit to client, based on experience
