Physical Therapist PT Per Diem
About the role
As a Physical Therapist, you will work closely with patients to develop and implement individualized treatment plans designed to improve their physical function and mobility. You will assess patients' physical abilities and needs, and design and implement a variety of interventions to address these needs. You will work collaboratively with other members of the healthcare team to ensure that patients receive comprehensive and effective care. You will maintain accurate and up-to-date records of patient care, including treatment plans, progress notes, and discharge summaries. You will participate in quality improvement initiatives and contribute to the development of best practices in physical therapy.
Responsibilities
- Conduct initial and ongoing assessments of patients' physical abilities and needs.
- Develop and implement individualized treatment plans, including goals, objectives, and interventions.
- Provide physical therapy interventions, including exercise programs, therapeutic activities, and education.
- Monitor and document patient progress, adjusting treatment plans as needed.
- Collaborate with other healthcare professionals to develop and implement comprehensive care plans.
- Participate in quality improvement initiatives and contribute to the development of best practices in physical therapy.
