Per Diem Orthopedic Physical Therapist

About the role

Provide orthopedic home physical therapy with a focus on patients recovering from total hip and knee replacements and other orthopedic conditions.

Responsibilities

• Evaluate patients to determine functional limitations and needs
• Develop and implement personalized treatment plans
• Educate patients and caregivers on safe mobility and fall prevention
• Assess discharge readiness and ensure safe transitions
• Document care accurately and maintain records using EMR systems
• Coordinate with interdisciplinary care teams
• Deliver functional training for activities of daily living
• Monitor patient progress and modify treatment plans as needed

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy
• Current state licensure and CPR certification
• Experience in orthopedic rehabilitation, with emphasis on joint replacements
