Per Diem Hospice RN - Day Shift

About the role

Provides in-home nursing care by planning, organizing, and managing patient services to meet individual needs. Collaborates with care teams and families to develop and implement care plans, conducts comprehensive assessments, and adjusts care as needed. Documents patient information accurately and timely, educates patients and caregivers on health management, and ensures compliance with legal and organizational standards. Performs on-call duties as required, demonstrating professionalism and adherence to safety protocols.

Responsibilities

• Assess patient needs and develop individualized care plans
• Provide safe, quality nursing care both directly and indirectly
• Coordinate with other healthcare disciplines and update care plans regularly
• Educate patients and families on health management and preventive care
• Complete all documentation following policies and legal requirements
• Participate in on-call duties as necessary
• Adhere to safety, legal, and organizational standards and policies

Requirements

• Valid RN licensure
• CPR/BLS certification
• Reliable vehicle and valid driver’s license

Qualifications

• Preferred Bachelor’s Degree in Nursing
• Experience in acute or home care

Skills

• Strong assessment, communication, and documentation skills
• Ability to work independently and as part of a multidisciplinary team
• Knowledge of patient rights, healthcare standards, and confidentiality policies
