Role Overview
Plans, organizes, and directs home care services to meet patient needs, managing caseloads efficiently. Provides independent patient care while collaborating with multidisciplinary teams to achieve care goals. Acts as a resource for patients, families, and care team members.
Qualifications & Skills
- RN licensure in the practice state (required)
- CPR/BLS certification (required)
- Valid driver’s license, reliable vehicle, and insurance (required)
- Bachelor’s Degree in Nursing (preferred)
- Experience in acute care or home care (preferred)
Responsibilities
- Conducts comprehensive initial and ongoing patient assessments, identifying issues and nursing diagnoses.
- Implements and updates individualized care plans in collaboration with patients and teams.
- Documents patient care, responses, and treatment outcomes accurately and timely.
- Provides patient and family education, counseling, and preventative care.
- Ensures compliance with legal, ethical, and organizational standards, including HIPAA and documentation policies.
- Performs on-call duties as required, supporting patient needs outside regular hours.
- Adheres to safety protocols, maintains professional conduct, and demonstrates service excellence.
- Must comply with vaccination requirements and participate in organizational policies, including E-Verify.
- Performs other related duties as assigned.
