Per Diem Family Nurse Practitioner or Physician Assistant

About the Role

This is a per diem, hourly primary care provider role involving patient assessments, treatment of acute and chronic conditions, and occasional tele-health visits. The provider collaborates with in-office and virtual teams, utilizing clinical training to perform procedures and deliver human-centered care.

Requirements

• Minimum 2 years of primary care experience
• Licensed or eligible for licensure in Connecticut
• Board certified or board eligible in Family or Internal Medicine
• Completed NP or PA program
• National certification
