About the Role

This position involves providing primary care services to patients, including acute, chronic, and wellness visits, both in-office and via telehealth. The role emphasizes collaboration with in-person and virtual teams, utilizing clinical training for patient treatment and procedures.

Responsibilities

• Conduct patient assessments and deliver appropriate care
• Collaborate with team members through daily huddles and virtual meetings
• Utilize clinical skills and perform in-office procedures when needed

Requirements

• Minimum 2 years primary care experience
• Current license or ability to obtain licensure in Connecticut
• Board certification or eligibility in Family or Internal Medicine
• Enrolled in or completed an accredited residency program
