About the Role

Provides assessment of and brief targeted intervention for social, emotional, and environmental issues that impact a client/family's ability to optimally benefit from care. Actively collaborates with the medical team to meet patient care management and system improvement goals. Provides leadership for department committees/workgroups and contributes to the plan of care with knowledge of population-specific needs related to patient psychosocial needs.

Vanderbilt Children’s University Pediatrics at One Hundred Oaks is committed to building ongoing relationships to address children’s medical needs as they grow and develop. The goal is to keep children well and thriving through comprehensive medical care from birth through adolescence, including regular sick and well visits. Same-day access is offered, linked to the child’s complete medical record to ensure continuity of care. The team strives to provide the highest quality care using up-to-date, evidence-based scientific information and expertise.

Location and Schedule

Position is located at One Hundred Oaks with preferred hours of 8:00 AM – 5:00 PM.

Responsibilities

• Maintains professional documentation that meets department and VUMC standards for compliance.
• Provides brief targeted interventions for identified clients.
• Completes psychosocial assessments and assists the team with the development of the plan of care.
• Provides education and/or consultation to clients, families, treatment teams, and the community.

Qualifications

• Experience with children (required).
• Experience within a medical setting (preferred).
• Master’s degree.
• 1 year of relevant work experience.

Skills and Technical Capabilities

• Crisis intervention in practical applications of moderate difficulty.
• Risk management of moderate difficulty, including risk analysis, probability, impact, early detection, and prevention.
• Preparation of simple contingency plans and alternatives.
• Patient assessments and evaluations in practical applications of moderate difficulty.
• Clinical counseling in practical applications of moderate difficulty.
• Assessment of the relative merits of alternative products or services.
