About the Role
Provides assessment of and brief targeted intervention for social, emotional, and environmental issues that impact a client/family's ability to optimally benefit from care. Actively collaborates with the medical team to meet patient care management and system improvement goals. Provides leadership for department committees/workgroups and contributes to the plan of care with knowledge of population-specific needs related to patient psychosocial needs.

Same-day access linked to a child’s complete medical record ensures continuity of care. Location and Schedule
This position is located at One Hundred Oaks, with preferred hours of 8 AM to 5 PM.
Preferred Qualifications
- Experience with children (required)
- Experience within the medical setting (preferred)
Responsibilities
- Maintains professional documentation that meets department and VUMC standards for compliance.
- Provides brief targeted interventions for identified clients.
- Completes psychosocial assessments and assists the team with the development of the plan of care.
- Provides education and/or consultation to clients, families, treatment teams, and the community.
Technical Capabilities
- Demonstrates crisis intervention in practical applications of moderate difficulty.
- Demonstrates risk management of moderate difficulty and conducts risk analysis, including determining risk factors, probability, impact, early detection, and prevention.
- Prepares simple contingency plans and alternatives.
- Conducts patient assessments and evaluations in practical applications of moderate difficulty.
- Effectively assesses the relative merits of alternative products or services.
- Demonstrates clinical counseling in practical applications of moderate difficulty.
