A 3-year-old with Trach/Vent, G-tube and Oxygen with continuous pulse ox.
Role Overview
We are seeking motivated LPN's with experience in Pediatric Home Health to join our team in Fruita.
Key Responsibilities
- Provide private duty nursing care to pediatric patients in their homes
- Administer medication, treatments, and therapies as prescribed by the physician
- Maintain accurate and complete medical records using web-based electronic medical records system
- Monitor patient's condition and report any changes to the healthcare team
- Assist with activities of daily living and provide emotional support to patients and their families
- Collaborate with other healthcare professionals to ensure optimal patient care
Requirements
- Active LPN License
- Current BLS Certification
- Valid Driver's License and Vehicle Insurance
- Strong time management and prioritization skills
- Excellent assessment skills and attention to detail

Pay
- Full-Time: $25.00 - $30.00 an hour
- Sign-on Bonus: $3,000.00. First half paid at 60 days of employment and second half paid at 120 days of employment of working expected hours.
- Part-Time: $1,500.00. First half paid at 60 days of employment and second half paid at 120 days of employment of working expected hours.
