Pediatric Home Health Nurse - High Acuity

Role: Pediatric LVN providing high-acuity skilled nursing care in patients' homes. Responsibilities Deliver hands-on clinical care to medically fragile children, including monitoring complex conditions.Manage ventilators, feeding tubes, and administer medications as ordered.Collaborate with RNs, physicians, and families to follow individualized care plans.Document patient information accurately in EMR systems.Respond promptly to changes in patient status, ensuring safety and quality care. Qualifications & Skills Licensed Vocational Nurse (LVN) certification.Experience in pediatric or high-acuity nursing preferred.Strong clinical skills in managing complex medical needs.Excellent communication and teamwork abilities.Ability to work independently and respond quickly to patient needs.
