Pediatric Care Manager RN
About the role
The RN Care Manager is an experienced Registered Nurse who serves as a core member of a multidisciplinary team, facilitating communication and coordination of patients' healthcare needs among primary care providers, specialty providers, healthcare facilities, community resources, and payers.
The RN Care Manager coordinates and supports care for pediatric patients with complex medical and/or psychosocial needs within pediatric clinics, oversees transitions of care, and facilitates referrals to specialty services, clinically based programs, and community resources. Through comprehensive care management, the RN Care Manager assesses patient and family needs, develops individualized care plans, promotes patient and family engagement and self-management, identifies and addresses barriers to care, and monitors progress toward established health goals.
The RN Care Manager collaborates closely with patients, families, and the healthcare team to ensure timely access to services, improve care coordination, and support optimal health outcomes across the continuum of care.
Responsibilities
- Coordinate and support care for pediatric patients with complex medical and/or psychosocial needs.
- Oversee transitions of care and facilitate referrals to specialty services, clinically based programs, and community resources.
- Assess patient and family needs and develop individualized care plans.
- Promote patient and family engagement and self-management.
- Identify and address barriers to care.
- Monitor progress toward established health goals.
- Collaborate with patients, families, and the healthcare team to ensure timely access to services and improve care coordination.
Requirements
- Graduate from an accredited school of Nursing.
- Washington Registered Nurse License upon hire.
- 3 years of clinical nursing experience.
- 1 year of Care Management or similar experience.
Qualifications
- Associate's Degree in Nursing (preferred).
- Bachelor’s Degree in Nursing (preferred).

Pay
The pay range for this position is $44.61 - $69.26 per hour. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.

Full time, Day shift.
