About the role
The Transitional Care Management RN works collaboratively within the Integrated Health Care for the Homeless team. The RN is responsible for coordinating health care transitions for patients of The Colorado Coalition for the Homeless (CCH), working with partner hospitals to improve patient outcomes with hospital discharges and access to hospital follow-up visits at CCH.
Responsibilities
- Assess patient needs and goals and assist in connecting patients to the correct resources.
- Provide comprehensive health care to all patients, including administering medications safely and effectively as per protocols.
- Facilitate access to internal programs such as health outreach and housing support services.
- Oversee care for patients with transitional, chronic, and complex health conditions.
- Mentor new nurses and nursing students, enhancing their clinical skills and confidence.
- Empower patients by providing appropriate health education and information, including managing chronic and acute health conditions.
- As a member of an OBH licensed Substance Use Disorder (SUD) treatment team, provide treatment services such as case management, crisis services, initial intake program screenings, individual medication assisted treatment (MAT) counseling, and administering scales to assess withdrawal.
- Precept to new nurses and nursing students.
Requirements
- AD, BSN, or MSN in nursing from an accredited college or university.
- Current licensure from the State of Colorado to practice as a Registered Nurse.
- Successful completion of privileging and credentialing process for RNs at the Coalition.
- Preferred experience in ambulatory, public, or community nursing.
- Preferred experience in mental health, emergency room, urgent care, palliative, care management, or critical care.
Qualifications
- Summary: AD, BSN, or MSN in nursing from an accredited college or university.
- Current licensure from the State of Colorado to practice as a Registered Nurse.
- Success in completing the privileging and credentialing process for RNs at the Coalition.
- Prior experience in ambulatory, public, or community nursing is preferred.
- Prior experience in mental health, emergency room, urgent care, palliative, care management, or critical care is preferred.
Skills
- Ability to perform quick and accurate triage assessments with appropriate recommendations of care as outlined in SSHC nursing protocols via telephone or in-person.
- Ability to work day, evening or weekend clinic as scheduled or appropriate.
