About the role

As an RN in the Transitional Care Center, you will provide compassionate care to patients and families, ensuring they have access to the necessary resources and support to recover effectively. This role requires strong clinical skills, excellent communication abilities, and the ability to work collaboratively within a multidisciplinary team.

Responsibilities

• Provide direct patient care, including monitoring vital signs, administering medications, and performing procedures as directed by a physician.
• Collaborate with other healthcare professionals to develop and implement individualized care plans for patients.
• Ensure patient safety by following all hospital policies and procedures related to infection control, medication administration, and emergency response.
• Communicate effectively with patients, families, and other healthcare providers to ensure clear understanding of treatment plans and discharge instructions.

Requirements

• Current licensure as a Registered Nurse in the state of Ohio.
• Bachelor’s degree in Nursing or related field required.
• At least one year of acute care nursing experience preferred.

Qualifications

• Excellent interpersonal and communication skills.
• Ability to work independently and as part of a team.
