About the role

Imagine Pediatrics is a tech-enabled, pediatrician-led medical group focused on delivering 24/7 virtual and in-home care for children with special health care needs. We work alongside families, providers, and health plans to enhance existing care teams and reduce unnecessary medical visits and hospitalizations.

Responsibilities

  • Manage a panel of complex pediatric patients
  • Oversee care plans, collaborate with PCPs and specialists, and ensure comprehensive care coordination
  • Work in a multidisciplinary team, including pediatricians, nurses, pharmacists, dietitians, psychiatrists, social workers, and therapists
  • Empower families to navigate their child's health journey with confidence, ensuring long-term health success
  • Deliver high-quality virtual care, conducting virtual enrollment visits, acute escalations, sick visits, post-hospital follow-ups, and in-home virtual assessments supported by the Mobile Integrated Care team
  • Step in when needed to bridge care gaps, coordinate prescriptions, and streamline referrals, removing barriers to care

Requirements

  • Minimum 5 years' experience with a pediatric population, with a preference for Community Based Health or Complex Care experience
  • Completed an accredited program for Nurse Practitioner (NP) or Physician Assistant (PA-C) and hold an unrestricted NP or PA state license, including board certification
  • Active NJ or NC licensure is required, with an additional license in FL, NY, GA, or DC preferred
  • Experience and comfort with children with complex medical needs
  • Experience with Medicaid and/or Community health preferred
  • Virtual care experience a plus

Qualifications

  • Strong desire to improve the quality of life for pediatric patients and their caregivers
  • Ability to lead and guide a clinical team
  • Bilingual Spanish is strongly preferred
