Pediatric Advanced Practice Provider

About the role

Imagine Pediatrics is a tech-enabled, pediatrician-led medical group focused on delivering 24/7 virtual and in-home care for children with special health care needs. We work alongside families, providers, and health plans to enhance existing care teams and reduce unnecessary medical visits and hospitalizations.

What You’ll Do

• Manage a panel of complex pediatric patients
• Oversee care plans, collaborate with PCPs and specialists, and ensure comprehensive care coordination
• Work in a multidisciplinary team to provide wrap-around care in a virtual-first model
• Empower families to navigate their child’s health journey with confidence
• Deliver high-quality virtual care, including virtual enrollment visits, acute escalations, sick visits, post-hospital follow-ups, and in-home virtual assessments
• Step in to bridge care gaps, coordinate prescriptions, and streamline referrals

What You Bring & How You Qualify

• Minimum 5 years’ experience with a pediatric population, with a preference for Community Based Health or Complex Care
• Completed an accredited program for Nurse Practitioner (NP) or Physician Assistant (PA-C) and hold an unrestricted NP or PA state license, including board certification
• Active LA or TN licensure required
• Experience and comfort with children with complex medical needs
• Experience with Medicaid and/or Community health preferred
• Virtual care experience a plus
• Strong desire to improve the quality of life for pediatric patients and their caregivers
• Ability to lead and guide a clinical team
• Bilingual Spanish is strongly preferred
