About the role
Imagine Pediatrics is a tech-enabled, pediatrician-led medical group focused on delivering 24/7 virtual and in-home care for children with special health care needs. We work alongside families, providers, and health plans to enhance existing care teams and reduce unnecessary medical visits and hospitalizations.
Responsibilities
- Manage a panel of complex pediatric patients
- Oversee care plans, collaborate with PCPs and specialists, and ensure comprehensive care coordination
- Work in a multidisciplinary team, including pediatricians, nurses, pharmacists, dietitians, psychiatrists, social workers, and therapists
- Empower families by educating and supporting parents to navigate their child's health journey with confidence
- Deliver high-quality virtual care, conducting virtual enrollment visits, acute escalations, sick visits, post-hospital follow-ups, and in-home virtual assessments supported by our Mobile Integrated Care team
- Step in to bridge care gaps, coordinate prescriptions, and streamline referrals to remove barriers to care
Requirements
- Minimum 5 years' experience with a pediatric population, with Community Based Health or Complex Care preferred
- Completed an accredited program for Nurse Practitioner (NP) or Physician Assistant (PA-C) and hold an unrestricted NP or PA state license, including board certification
- Active TX, LA, MO, or TN licensure required
- Experience and comfort with children with complex medical needs
- Experience with Medicaid and/or Community health preferred
- Virtual care experience a plus
- Strong desire to improve the quality of life for pediatric patients and their caregivers
- Ability to lead and guide a clinical team
- Bilingual Spanish is strongly preferred

Pay
The starting compensation for this role is $115,000, with an annual bonus and a competitive company benefits package. Eligibility to participate in an employee equity purchase program is also available.
Schedule
Travel is estimated to be up to 10%, and the expected schedule requirements are 4x10s (8:00 am - 6:00 pm) Monday-Friday local time.
