About the Role
The Patient Transfer Facilitator coordinates the placement of all admissions and transfers to UK Chandler / Good Samaritan in accordance with the clinical standard guidelines. Is responsible for ensuring that the patient is admitted to the most appropriate bed available using current admission criteria. Follows protocols for direct acceptance of specific patient population.
Responsibilities
- Providing total, comprehensive, continuous patient centered care which includes: assessing, documenting, planning, implementing, teaching, developing, and evaluating a comprehensive nursing plan of care
- Delivering direct patient care
- Ensuring patient satisfaction
- Adhering to University policies and procedures as well as federal, state and local regulations
- Utilization of the nursing process in conjunction with a comprehensive patient-centered plan of care
- Supporting collaborative efforts among the nursing staff and the team of physicians to increase communication and enhance expected patient outcomes
Requirements
- Required Education: ADN
- Required Related Experience: 4 years
- Required License/Registration/Certification: Registered Nurse (RN)
Qualifications
- ICU Experience Preferred
- BSN Preferred
Pay
$29.12-45.70/hour
Schedule
Must be able to work at least 36 hours per week from 7pm until 7:30am and may be required to work some weekends and/or holidays as needed by the department.
