Patient Referral Coordinator

About the role

Assist in the transition of patients from the acute care setting to aftercare facilities, agencies or programs in the community in collaboration with the multidisciplinary team, payers and community providers. Provide information and assistance to internal and external customers on aftercare programs and facilities.

Responsibilities

• Coordination of paperwork and discharge documents
• Communication of requests for review
• Maintenance of reports and statistics pertaining to aftercare placement activities

Requirements

• High School Diploma
• Three years experience in a hospital setting or related health field
