Patient Referral Coordinator

About the role
We are a nonprofit organization with a long history of serving older adults and their families across South Florida. Our team is expanding and we are looking for caring, detail-oriented professionals who want to build a career with purpose.
Responsibilities
- Handle 40-60 inbound calls daily from patients and their caregivers.
- Collect and verify demographic and insurance information.
- Identify the right resources and connect clients to the services and support they need most.
Requirements
- Bachelor's degree in human services or a related field.
- Working knowledge of Medicaid and Medicare processes.
- 2+ years of call center experience.
- Bilingual: English/Spanish or English/Creole.
- A warm, professional communication style with genuine empathy.
- Proficiency in Microsoft Office Suite and Microsoft Teams.
- Typing speed of 35+ WPM.

Schedule
Hybrid (3 Days In Office, 2 Days Remote) Monday - Friday, 8AM - 5PM
