Patient Referral Coordinator

The Patient Referral Coordinator plays a vital role in ensuring patients are ready for care — clinically, administratively, and financially — before services begin. By coordinating referrals accurately and efficiently, this role helps remove barriers to treatment and supports a smooth, patient-first experience.
What You’ll Do
- Ensure patient referrals are fully prepared for service by gathering required documentation, confirming insurance coverage, and securing authorizations in compliance with all regulatory requirements

- Coordinate new infusion and enteral referrals from intake through initial processing, ensuring accuracy, timeliness, and clarity at every step

- Collaborate closely with referral sources, hospital liaisons, pharmacy, nursing, billing, and branch teams to support seamless care delivery

- Communicate coverage details and financial obligations clearly to patients or their representatives, ensuring understanding and agreement prior to service

- Maintain accurate patient records, referral logs, and authorization tracking to support timely billing and reduce delays in reimbursement

- Identify opportunities to improve intake and referral workflows while maintaining high standards of service and compliance

Qualifications
- An associate's degree in Business, Healthcare Services, or a related field required
- Minimum of one (1) year of healthcare experience required
- Strong knowledge of payer requirements related to billing, authorizations, and coverage
- Proficiency with computer systems and documentation accuracy
- Strong organizational, multitasking, and problem-resolution skills
Preferred
- Two (2) or more years of experience with a home care or infusion provider
- Prior home infusion experience on the provider or payer side
Pay Range/Rate
$22 - $26 USD
