Patient Service Representative - Community Care - Full Time - Foundation Medical Partners

About the Role

Affiliated with Southern New Hampshire Medical Center, Foundation Medical Partners is the second largest multi-specialty group in New Hampshire and serves the greater Nashua community. Our Home Care program is designed for patients who are unable to leave their home due to a medical condition. As a patient of Southern New Hampshire Health and Foundation Medical Partners, we offer Home Care visitation services to support patients’ care while medically necessary. Our Nurse Practitioner is available to visit patients upon discharge from the hospital or on an ongoing basis, working collaboratively with primary care, specialists, and patients’ care teams to provide care for specific needs. Our goal is to allow patients to remain in their homes while still receiving the important health care needed.

Responsibilities

• Greets patients.
• Answers telephone calls and provides general information.
• Checks in and checks out patients.
• Schedules appointments and enters and tracks referrals.
• Scans and indexes documentation.
• Communicates with internal and external customers.
• Performs filing and other duties as assigned.

Requirements

• High School diploma or equivalent required.
• Minimum of one year of experience in a customer service position preferred.
• Knowledge of reception tasks, clinic policies/procedures, and paperwork.
