About the role
The Patient Scheduler plays a vital role in a clinic-based care team, coordinating appointments for patients receiving palliative and dementia care. They serve as the primary contact for patients, families, and caregivers, providing compassionate communication and support, especially for those with serious illnesses or cognitive impairments.
Responsibilities
- Schedule and manage in-clinic and home visits for physicians and interdisciplinary team members
- Ensure efficient provider schedules
- Handle cancellations and same-day requests
- Document appointment details in the EMR
- Verify patient information
- Support administrative workflows
- Collaborate with healthcare staff
Requirements
- High school diploma or equivalent
- 1-3 years healthcare scheduling experience
- Proficiency in EPIC and office software
- Strong organizational and communication skills
- Ability to develop positive relationships with diverse populations
- Physical, mental, and sensory skills essential for the role
- Ability to handle potential hazards in a healthcare environment
Schedule
Monday through Friday, with potential after-hours or weekend calls.
