Patient Navigator - Washington St. Poughkeepsie

About the role

Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in multiple counties including Suffolk, Rockland, Orange, Duchess, Ulster, Sullivan, Columbia, and Westchester. We are seeking a Full-time Patient Navigator to support our Washington St. - Poughkeepsie, NY site.

Responsibilities

• Consult with medical staff to link patients to services through the provision of ancillaries such as translation, transportation assistance and application assistance.
• Facilitates patient appointments within different departments of SRH and outside agencies.
• Follows SRH procedures for referring patients to different departments of SRH and outside agencies, utilizing SRH preferred provider lists.
• Aid patients in overcoming obstacles that would prevent them from receiving needed referrals.
• Facilitate patients to appropriate referred agencies, with specific focus on referrals to specialty care and public health insurance programs, as needed and appropriate; track progress in eCW and alert providers.
• Answer patient questions to clarify provider instructions, procedures and referral needs.
• Transmit all necessary clinical information available to specialists for referral; obtain consultant reports; obtain consent for release of information.
• Review open referrals for upcoming medical appointments and provide to clinical team.

Requirements

Bilingual Spanish is preferred.

Qualifications

• High school diploma/GED
• 0-2 years of work-related experience
