About the role
Serve as a critical bridge between the hospital and the community.
Facilitate resource navigation for patients during their emergency department visit.
Refer complex clinical needs to Behavioral Health or RN Care Managers.

Requirements
- High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Bachelor's degree OR 4 years of applicable cumulative job specific experience required.
Additional Preferences
- Strong understanding of community and public resources available to patients who are on Medicaid or uninsured.
- Must be able to engage patients, build trust quickly, and assess their level of motivation and need.
- Demonstrate sensitivity to diverse populations and an understanding of unique cultural barriers to care.
- Excellent communication skills and the ability to refer patients to appropriate Case Managers for continued care.
