Patient Navigator/Medical Case Manager (grant-funded) - Jefferson HIV Program - New Jersey

Location: 333 Laurel Oak Road, Voorhees, New Jersey, USA. Regular employee with Kennedy Medical Group, Practice, PC.

Summary

The Patient Navigator serves as a liaison for Jefferson patients, working with physicians and staff to determine appropriate services, schedule appointments across multiple departments, and follow up to ensure compliance. The role supports the Jefferson Experience by emphasizing prompt and courteous treatment of all patients and visitors.

Responsibilities

• Interact with co‑workers, visitors, and other staff consistent with Jefferson values.
• Schedule patients for all visits undergoing future care.
• Administer screenings and intake assessments as needed.
• Monitor treatment plan adherence.
• Triage patient phone calls.
• Coordinate inpatient care as needed.
• Develop, maintain, and update community resource list.
• Greet and interact with patients/visitors professionally, on phone or in person.
• Adhere to HIPAA privacy and security requirements.
• Ensure compliance with all applicable federal, state, and local regulatory standards.
• Perform other duties as requested or assigned.

Minimum Qualifications

• 3 years of experience in a medical office.
• Ability to communicate courteously with patients and/or visitors.
• Willingness to provide and support a positive Jefferson Experience.
• Leadership skills including proactivity and problem solving.
• Ability to establish and develop positive and collaborative relationships.

Preferred Qualifications

• Bachelor’s degree in psychology, health administration, or related field from an accredited college/university.
• Epic experience preferred.

Additional Qualifications

• Physical demands: lift and carry 40‑50 lb; push/pull patients on bed or stretcher (requires 29 lb push force); lateral transfers up to 50 lb of patient weight.
• Frequent to continuous standing/walking; patient transporters can walk 8‑10 miles per shift.
• May be required to kneel, bend, squat, and perform CPR as role‑specific activities.

Pay

Salary range $20.58 to $29.34 hourly. Actual hiring rate determined based on candidate experience, skills, and qualifications. Position not eligible for an annual incentive.
