Primary location: Shea Medical Center, 9003 E. Shea Blvd, Scottsdale, AZ 85260. This role is hybrid, requiring travel to various Cardiovascular Navigation locations. Hours are Monday–Friday, 8:00 am–5:00 pm.

About the role

This position acts as a resource for patients, families, and caregivers, coordinating and guiding patients through a comprehensive plan of care to overcome healthcare system barriers, facilitate timely access to quality health and psychosocial care from diagnosis through all phases of the plan.

Responsibilities

  • Expedites, initiates, participates, completes, tracks, and updates the patient’s plan of care.
  • Communicates effectively with primary care physicians, hospitals, specialists, and post-acute care facilities to schedule appointments and identify and fill gaps in care.
  • Provides follow-up with participating providers to ensure patients follow through with scheduled appointments.
  • Provides immediate communication with all referring physicians regarding their patients’ results and coordinates physicians’ requests for care.
  • Provides educational and research support, including contacts for upcoming appointments, business cards, directions, and patient preparation.
  • Educates patients on the availability of resource libraries, psycho-social support, treatment resources, community resource support, family educational resources, and financial assistance.
  • Coordinates all necessary referrals for identified services in the patient’s plan of care.
  • Conducts outreach and patient follow-up using call scripts and pre-defined processes.
  • Schedules exams and other screening services; facilitates insurance verification and communicates with referring physicians regarding patients’ upcoming exams or concerns.
  • Maintains patient data using various tools and quality reporting processes/documentation.
  • Performs other duties as assigned.

Requirements

  • High School Diploma or GED required.
  • 3 years of experience working within a health insurance call center dealing with benefit verification and scheduling or similar experience in a healthcare setting required; 5 years preferred.
  • Experience with the Cardiovascular patient population preferred.
