About the role
UAB St. Vincent’s, a part of UAB Medicine, is a long-standing provider of healthcare in Alabama. The department offers a range of services aimed at improving the health and lives of its patients.
Responsibilities
- Cook up access to health-related services by helping clients utilize available resources.
- Identify potential clients, conduct initial needs assessments, and follow up to provide appropriate guidance to health services.
- Offer informal counseling, education, and social support to clients.
- Aid clients in accessing necessary resources, such as scheduling medical appointments, accompanying them to appointments, and helping with medical program applications.
- Build and sustain strong working relationships with external referral sources and community agencies, as well as internal contacts within the department.
- Keep detailed records of all client interactions, services provided, and outcomes to track progress and future reference.
Requirements
- A high school diploma equivalency with at least 2 years of cumulative experience.
- An associate's degree or bachelor's degree.
- 4 years of relevant work experience.
