Patient Navigator - Belvidere and Stanhope, NJ

About the role

Crossroads Treatment Centers is a family of professionals dedicated to providing the most accessible, highest quality, evidence-based medication assisted treatment (MAT) options to combat the growing opioid epidemic and help people with opioid use disorder start their path to recovery. Our clinics are all outpatient and office-based, with locations in Georgia, Kentucky, New Jersey, North and South Carolina, Pennsylvania, Tennessee, Texas, and Virginia.

Responsibilities

• Greet patients and answer clinic phones.
• Assist patients in understanding their treatment options and resources.
• Serve as an essential link between patients and all other care.
• Facilitate appointments for consults and support services within established service standards, including social workers, physicians, etc.
• Coordinate multidisciplinary planning conferences, develop concise patient summaries for use by the care team, and document recommendations made utilizing standardized care protocols in accordance with nationally recognized care guidelines.
• Follow patients throughout the care continuum, including inpatient admissions, and collaborate with inpatient care management.
• Work with staff and others to collect data, track outcomes, and support strategic planning processes.
• Responsible for outreach efforts to establish and maintain positive working relationships with physicians, office staff, nurses, and social services.
• Assist Center in stocking supplies and Patient Ambassador team.
• Other duties as assigned.

Requirements

Must hold one of the following credentials/experiences:

• RN, LPN, or BSW
• Bachelor’s Degree WITH at least 2 years of lived addiction experience.
• Associate's degree or CMA WITH 4 years of lived addiction experience.

Schedule

• Monday (Belvidere): 8:30am–1:30pm
• Wednesday (Stanhope): 9:30am–2:00pm
• Thursday (Belvidere): 11:30am–5:30pm
• Sunday (Stanhope): 9:00am–1:00pm
